Birth Doula Service Description and Agreement (Microsoft Word form)
Please sign this and return it to me to hire me as your birth doula. First, I do require we meet and that we all (mother, partner and I) feel we are a good fit, and that my schedule is open (I take one birth a month). Give me a call or email me and we can meet in your home or in a public place.
Birthing From Within Childbirth Class Registration Form (Microsoft Word form)
Please fill out this form to sign up for childbirth education services. You can either email it to me filled out for faster, cheaper, more environmentally-friendly service, or you can mail it to me. Either way, please include a non-refundable deposit of $50 to hold your space in class. (If you send your registration form via email, I’ll bill you via PayPal.)
